Your Visit

Before coming to the visit:

Make sure you have a valid Canadian health card. 

• Make sure you have done the investigation (lab. Work, imaging studies, etc…you were supposed to do, if not please let our office know and we can schedule your appointment

• Make sure you have any updated health info which pertain to your condition since we have seen you last.

 Before your phone visit:

• Find a private location to do the phone interview with the doctor without being disturbed.

• Please have your health card handy during the interview.

• Please have all your medications handy during the interview. Please have your recent readings of BP/ HR/ Wt./BG if possible. you can also send them before the appointment securely to us here.  

• If needed, and if you have not done so already, Please remember to update us about any recent hospital admission/visit since your last visit with us or any recent change in medications since last visit here  

 Before your video visit:

• Find a private location to do the phone interview with the doctor without being disturbed.

 • Use a private computer/device (i.e., not an employer's or third party's computer/device), secure accounts, and a secure internet connection. For example, using a personal and encrypted email account is more secure than an unencrypted email account, and your access to the Internet on your home network will generally be more secure than an open guest Wi- Fi connection.

 During your  physical or virtual visit:

• Please remember to ask for medications renewal during your appointment. If you forget to do so during your regular appointment, you will need to make an appointment, physical or virtual (phone or video) to renew your medications, but this appointment can not be guaranteed to be on time before you run out of your medications supply.

• Please tell us if you would like your lab request to be emailed to you, or you should pick it up before you leave the office. Sometimes it can be faxed directly to the lab of your choice. Please ask the reception.

 Before you leave the office:

• Before you leave the office please remember to have your prescriptions renewed on your office visit. We do not renew by telephone or fax. You have to request a video or phone visit with the doctor to review your medications and renew them if you forgot to ask for renewal during your actual visit. Medical conditions change over time, before we renew your medications we would like to know the most updated information about your health. 

 • Please check at reception for your next appointment and any test appointments that may need to be made before you leave the office.

After your visit:

• We randomly send patient satisfaction survey after the clinical visit, as we would like to know what you think about the services we provide. If you get one of these, please respond so we can make sure we are meeting your needs and improve our work.

 

Clinical Visit

Bring your health card, medications, BP, BG readings. Prepare issues you would like to discuss with your doctor. Upload info here before visit if needed.

Echocardiogram 

There is no special preparation for the test. You can take all your medication as normal. You can eat and drink before and after the test as normal. More info.

Exercise Stress Test 

Do not eat, smoke or drink alcohol or caffeinated beverages for at least 4 hours before the test. More Instructions.

Holter Monitor

The recording monitor will be started by technician, and you will be given instructions on how to replace electrodes should they become loosened.More Instructions.

Myocardial Perfusion Scan

Do not eat, smoke or drink alcohol for at least 4 hours before the test. Avoid caffeine for at least 12 hours before the test. This includes: tea and coffee, all sodas, even ones that are labeled caffeine-free, chocolates, certain pain relievers that contain caffeine. More Info.

Ambulatory BP Monitor 

Do not drink any coffee or other caffeinated beverages for an hour before coming to the appointment. Wear loose, comfortable clothing and a waist belt to attach the monitor. The recording monitor will be set up and started by the technician, and you will be given instructions on what to do during and after the monitoring period. More info.

Spirometry

Don’t smoke or do strenuous exercise on the day of the test • don’t drink alcohol or eat a big meal for 4 hours before the test • wear comfortable, unrestrictive clothing for the test • if you use puffers or inhalers try not to use the reliever ones for at least 4 hours before the test • ensure that your bladder is empty • remove any loose fitting dentures. ECG

There is no special preparation for the test. You may wear a loose-fitting blouse or shirt. Avoid wearing jewelry or clothes with metal buttons or buckles, because these can interfere with the recording. More Info.
